Glass slides stained with hematoxylin and eosin are required, along with a pertinent summary of clinical findings, and a contact telephone number and FAX number.  A paraffin block is highly desirable as well, or, alternatively, 10-12 unstained histologic sections mounted on adhesive slides. Paraffin blocks and original slides will be returned promptly after completion of the consultation, and a FAXed report will be sent to the submitting physician.

Either the referring physician or the patient may be billed for Dr. Patterson's services (at a rate of approximately $150, plus costs of adjunctive studies).  However, if direct patient billing is desired, complete demographic and insurance information must be supplied by the submittor.
